The Why and How of Changes Office and Other Outpatient Evaluation and Management Services 53 The Why and How of Changes to Office and Other Outpatient Evaluation and Management Services The Why and How of Changes to Office and Other Outpatient Evaluation and Management Services Current Procedural Terminology (CPT ®) 2021 will bring revised codes and guidelines for code ­ selection for evaluation and management (E/M) services in the office and other outpatient settings. These changes will only affect services on and after January 1, 2021. Background Since October 2017, the Centers for Medicare & Medicaid Services (CMS) has, as part of the ongoing Patients Over Paperwork initiative, taken actions intended to reduce the paperwork burden on physicians. As part of this effort, the CMS has sought to reduce the burdens associ- ated with documentation and code selection for office and other outpa- tient E/M services. Key Changes Expected in 2021 Effective for dates of service on and after January 1, 2021 Code 99201 will be deleted from CPT. Code descriptors for codes 99202–99205 and 99212–99215 will include options to select codes based on the level of MDM or total physician or other qualified health care professional time on the date of service (ie, not just face-to-face time with the patient). History and examination elements will no longer be factors in ­ determining the level of service. CPT will include separate E/M guidelines for office and other out­patient E/M codes and for other E/M services. Code 99211 will remain for reporting E/M services by clinical staff.
